How it works
Chlorpromazine is a phenothiazine neuroleptic. It depresses activity in the central nervous system and blocks dopamine receptors in the brain, which helps calm psychotic symptoms. It also has anti-sickness, sedative and blood-pressure-lowering effects, which is why drowsiness, dry mouth and dizziness can occur. The liver processes it and it leaves the body in urine and bile.
Uses
Schizophrenia, psychosis and mania: the UK product information lists schizophrenia and other psychoses, especially where paranoia is a main symptom, as well as mania and hypomania.
Anxiety, agitation and impulsive behaviour: chlorpromazine may be used as a short-term add-on treatment for severe anxiety, agitation, excitement and dangerously impulsive behaviour.
Other UK-listed uses: persistent hiccups, nausea and vomiting in terminal illness when other medicines have failed, lowering body temperature, and childhood schizophrenia and autism.
In Australia, Healthdirect lists Largactil for acute psychosis, long-term schizophrenia, short-term agitation, severe behavioural disturbance in some children, symptom control in terminal illness and intractable hiccups. It relieves symptoms rather than removing their cause. Check your pack label.
Dosage and how to take it
The UK leaflet says to take chlorpromazine exactly as your doctor or pharmacist has told you. It states a recommended adult dose of 40 mg to 300 mg a day in divided doses, with one third to one half of that for elderly or weak patients. Do not stop or change the dose unless your prescriber says so. Follow the leaflet in your pack or your prescriber's or pharmacist's advice.
Age restrictions vary by country. The UK leaflet says children under 1 year should not take this medicine, with a daily maximum of 40 mg for ages 1 to 5 and 75 mg for ages 6 to 12. The Australian consumer leaflet gives no children's age guidance, and the US label generally advises against use under 6 months unless it may be lifesaving.
Storage Instructions
The UK product information says do not store above 25°C. Keep the tablets in a dry place, protected from light, with the container tightly closed. Other countries differ: Australia says below 30°C and the US label says 20°C to 25°C. Keep out of the reach of children and do not use after the expiry date.
Possible side effects
- Drowsiness or sleepiness, especially at the start of treatment
- Dry mouth
- Weight gain
- Constipation
- Dizziness or light-headedness when standing up quickly
- Uncontrolled movements, tremor, muscle stiffness or restlessness
- Anxiety
- Missed periods or abnormal breast milk production
- Changes in heart rhythm, shown as QT prolongation on an ECG
- Skin that is more sensitive to sunlight
Serious reactions can include high temperature with stiff muscles and confusion, yellowing of the skin or eyes, frequent infections, easy bruising, blood clots, a long-lasting painful erection, severe allergic reactions and a serious rash. Seek urgent medical attention for these, for difficulty breathing or for a very fast or uneven heartbeat.
Safety
- Drowsiness and driving: the UK leaflet says not to drive or use tools or machines if you feel drowsy or your eyesight is affected.
- Alcohol: the UK leaflet says you must not drink alcohol, because it increases drowsiness and may cause serious breathing problems.
- Allergy and blood conditions: the UK leaflet says not to take it if you are allergic to phenothiazines or have bone marrow depression, a history of low white blood cells, glaucoma or prostate-related urine retention. Report fever, sore throat or infections at once.
- Dementia: not licensed for dementia-related behavioural problems (UK). The US label carries a boxed warning of increased risk of death in elderly people with dementia-related psychosis.
- Heart rhythm: chlorpromazine can lengthen the QT interval, so your doctor may arrange an ECG, blood tests and eye tests (UK leaflet).
- Interactions: the UK leaflet says not to combine it with citalopram, escitalopram or dopaminergic antiparkinsonism medicines. Tell your prescriber about all other medicines, including lithium and sedatives.
- Pregnancy and breastfeeding: the UK leaflet advises against use unless your doctor feels it is absolutely necessary, and says not to breastfeed.
- Liver, skin and sunlight: report yellowing of the skin or eyes, seek urgent help for a serious rash and keep out of direct sunlight.
- Movement problems and temperature: the Australian leaflet warns about tardive dyskinesia, especially with long-term use.
- Stopping and overdose: do not stop suddenly, as nausea, vomiting and sleep problems can follow. In an overdose, go to hospital straight away with the pack (UK leaflet).
